Position Summary: The League is seeking the right candidate for the position of Community Living Coordinator
This position is responsible to support individual consumers with institutional transition and diversion efforts. This staff person leads independent living skills training for individuals who have disabilities and provides peer counseling and support to consumers on the caseload. Additionally, this person conducts presentations and participates in individual and systemic advocacy activities.
The efforts of this position should result in: 1. serve a minimum of 65 active consumers per year, 2. achieve a program consumer goal accomplishment rate of at least 65%, 3. assist 30 consumers per year avoid institutionalization or return to the community from institutions, 4. assist 60% of those served each year increase their ability to access transporta-tion, housing, health care and/or adaptive equipment, 5. assist 70% of those served each year increase their ability to care for themselves, their family or household, and 70% of consumer will remain independent in their homes.

Education/Experience: Bachelor’s degree (B.A.) from a four-year college or university; or two years of post-secondary education and two years of related experience and/or training; or an equivalent combination of education and experi-ence. Prefer a qualified person with a disability.
